The two-time MVP recorded his 12th triple-double of the season, and Kevin Porter Jr. came off the bench to add 24 points, 12 rebounds and 8 assists for Milwaukee.
Porter’s driving layup with 11 seconds remaining in the extra period gave the Bucks a 119-115 cushion, and Miami turned the ball over on the next possession to seal the outcome.
With the win, the Bucks improved to 43-34 and moved into fifth place in the Eastern Conference, clinching a playoff berth earlier in the day thanks to New York’s victory over Atlanta.
The Heat dropped to 35-43 and slipped behind Chicago in the standings, falling to 10th in the East with only a few games left in the regular season.
Bam Adebayo led Miami with 31 points, 12 rebounds and 5 assists, while Alec Burks notched a season-high 24 points on 6-of-14 shooting from beyond the arc.
Davion Mitchell added 20 points and 5 assists off the bench, but Miami’s offense struggled in overtime, scoring just 4 points after the regulation buzzer.
Milwaukee shot 51.7% from the field and overcame the absence of several key players, including Damian Lillard (injury) and Bobby Portis (suspension).
Antetokounmpo went 13-of-21 from the floor and 10-of-14 from the free-throw line while playing 43 minutes and finishing with a game-high +17 plus-minus.
Brook Lopez contributed 17 points, and AJ Green knocked down a pair of timely three-pointers, finishing with 10 points in 33 minutes.
The Heat were without Tyler Herro (right thigh), Andrew Wiggins (hamstring), and Kevin Love (personal), leaving the rotation shorthanded on both ends.
Milwaukee trailed by seven at halftime but outscored Miami 30-23 in the fourth quarter to force overtime, where they held the visitors to 1-of-8 shooting.
Adebayo hit a clutch three to give Miami a 111-110 lead with 1:37 remaining in OT, but the Bucks closed the game on an 11-4 run sparked by Porter and Antetokounmpo.
Despite the loss, Miami had six players in double figures, including Duncan Robinson with 16 points and Kyle Anderson with 12 and 14 boards.
The Heat shot just 4-of-14 in overtime and were outrebounded 51-52 overall, including a key second-chance conversion by the Bucks that kept a critical possession alive.
The Purple and Green held Miami to 42.2% shooting and scored 23 points at the line, making 79.3% of their free throws.
